Home-Based Psychiatric Services: A Continuum of Care for Hospitals, Home Health Care, Community Care, and Physicians

This article addresses the development and implementation of a home-based psychiatric (psych) program as a continuum of care in conjunction with hospital care and physician care. Psych patients experience a high level of anxiety and stress; home psych- program coordination offsets this by directing a seamless system of health care delivery. Psych patients can transition from one level of care to another with fewer problems. In the current environment of regulatory and reimbursement cutbacks, improved utilization of resources is imperative to ensure long-term viability of all health care settings. The ability to case manage psych patients by meeting their health care needs in the setting of care at the right point is essential.
