Home total parenteral nutrition (TPN) presents many challenges to the home health care team, with the increase in patient acuity, advance in technologies, and increase in stringent guide lines set by insurance companies. The purpose of this article is to review the current state of home TPN, including specifying indications and contraindications for home TPN candidacy; assessing, instructing, and monitoring patients on home TPN; as well as discussing parameters for terminating home TPN. A well-managed home TPN program can provide benefits through decreased health care cost to society and increased quality of life for the patient.
