Case management of HIV-positive and AIDS patients

Elmhurst Hospital Home Care City Hospital Center at Elmhurst Queens, New York

Elmhurst Hospital Home Care, which is one of six municipal home health agencies within New York City's Health and Hospital Corporation, is hospital based and operates within the structure of Elmhurst Hospital Center. Elmhurst Hospital Center is a major treatment center for patients with acquired immune deficiency syndrome (AIDS) in the borough of Queens. Finding resources to service underinsured and uninsured AIDS patients and patients who are positive for the human immunodeficiency virus (HIV) in a concentrated geographical area was initiated through a grant proposal. Awareness of the complex care needs created a model design emphasizing a clinical and coordinating relationship with the infectious disease department. The model ultimately achieved all original goals.
