Collaboration: A key to future success in long-term home careVisiting Nurse Affiliates of Cambridge Cambridge, Massachusetts The new health care reform plan recognizes long-term care as a key component of care for the elderly, the disabled, and medically fragile children. As the need for this type of care continues to grow, collaboration, cooperation, and coordination of care by existing providers are essential. Through such efforts and a managed care approach, a comprehensive system for providing long- term care will emerge. This article describes in detail the House Calls program developed in 1989 to meet the needs of the homebound elderly.
