Diabetes education and home health care: A natural combination

Home health care nurses are diabetes educators. They are able to assess the educational needs of patients and families in the home environment and provide education that will affect the diabetic's life. This article presents important aspects of the assessment process, presents teaching plans, and provides the home health care team with resource informa tion for diabetes education standards and certification.
