Social work intervention with stroke patients and their families

Social work services in home care are provided to improve or maintain the social, emo tional, functional, and physical health status of the patient. Gill explains that the patient who has experienced a cerebrovascular accident (CVA) must receive an interdisciplinary team approach to care, and the social worker is a valuable member of this team. A case study points out the goals and effects of a well-thought-out and well-implemented plan of care.
