How Do We Know It's Working? Effective Intervention MonitoringQualis Health Home health agencies across the United States have been exploring techniques to reduce acute care hospitalizations. Many are promoting change in staff behaviors to implement best practice interventions. The frequently overlooked component of this change process is monitoring to ensure the interventions are being implemented and the changes are leading to improvements. In this article, the components of balanced intervention monitoring, including process and outcome, are provided with examples and rationale.
