Improvement in the Management of Oral Medications: Collaborative Approaches

This article describes two collaborative approaches that have proven to be effective in improving oral medication management in the home care setting. The first approach to be discussed is one that began with the introduction of best practice tools and supporting resources through the Quality Medication Administration Project collaborative. A case study highlights the experience of a home health care agency in the implementation of the Q-MAP tools and resources. The second approach describes a pharmacist's perspective of a patient-centered medication therapy program that promotes direct collaboration between home health agencies and qualified pharmacists. This article developed by a team of authors provides approaches and application examples to present a broad picture of the initiative.
