Phoenix and the Leaders: The Transformation of an Academic Health System-Based Home Care Provider

The Phoenix is the story of Johns Hopkins Home Care Group (JHHCG). This multiservice home care program, in operation since 1983, has undergone radical change in the past few years. This article focuses on the role of the leadership team in designing goals, setting priorities, and supporting communication to create the inspiration and focus that breed success. Detailed are the specifics of this provider's market, history, structure, and goals. Home health care for adult and pediatric patients, home medical equipment including clinical respiratory care, infusion pharmacy, ambulatory pharmacy, and hospice service are the service lines offered at the JHHCG. Readers can glean how leadership planned structure and process to achieve desired outcomes.
