A Future Vision of Innovative and Expanded Community Involvement in End of Life CareResearch has clearly identified that Americans do not die well. Many die alone, isolated and in pain in hospitals. Improvements will come only with greater individual and societal commitment. One way to ensure change is through expanded community involvement in end of life care reform. The Community-State Partnerships to Improve End-of-Life Care are serving as a model for building community awareness and involvement. Home health is in an opportune position to take a leadership role in reform by convening community coalitions and by bringing the home health philosophy and perspective to these coalitions.
