APN Elder Home Care: A Successful Model

Advanced practice nursing can play an important role in home care and particularly in elder care with the complexity of issues that exists. The author chronicles the care of an elderly female patient living in suboptimal conditions in an urban environment. By employing accurate assessments, appropriate interventions, and collaborative approaches, a model of care evolved that allows for an elder to remain safely in her home, avoid hospitalizations, and delay placement in a long-term care facility. Quality of life and quality of care are both positively affected with advanced practice nursing in elder care.
