Teaching Patients to Manage Diabetes Safely in the Home
Using an interdisciplinary process improvement team approach the average patient's diabetic knowledge improved 27% during the episode of care. This improvement was accomplished by creating a pre-/post- test that could also be used as a teaching tool.
