Home health nursing is a vital link between acute care and client wellness. Acute care provides treatment during the critical event, but it is home health that follows through and assures continued progress toward better health. Families and home care providers need the education and individual attention provided by nurses. In an era of multiple specialties the nurse is the central case manager. Patients and families can not sort out all of the information given to them without guidance. In this case study the nurse supports the family of a client newly diagnosed with chronic renal failure and anemia.
